Fix medication button visibility with collapsible form pattern (#543)

The addMedicationButton in MedicationListFragment was not reliably
findable by Maestro UI tests because the always-visible form consumed
too much vertical space. This applies the same collapsible form pattern
used by FoodLogFragment, SportLogFragment, MedicationLogFragment, and
other diary sub-pages: the form fields start hidden (visibility=gone)
behind a toggle button, and collapse again after successful submission.
