fix: provide DATABASE_URL fallback to prevent 'root' user connection

When DATABASE_URL is nil, Ecto falls back to system defaults which
uses the current user as database username. In Docker containers
running as root, this causes "role root does not exist" errors.

Added fallback to postgres:postgres@localhost for both:
- rel/config/prod.exs (Distillery release config)
- config/prod.exs (standard prod config)
