fix: add DATABASE_URL default to Dockerfile for migration timing

Ecto.Migrator.with_repo runs before Distillery config provider loads
runtime configuration. This causes migrations to fail with "role 'root'
does not exist" because DATABASE_URL is not yet available.

Adding defaults directly in Dockerfile ensures environment variables
are present during the entire container startup sequence.
