ci: consolidate CI/CD workflows

* ci: consolidate CI/CD workflows

- ci.yml: PR events only (lint, typecheck, build, test, docker verify)
- cd.yml: main pushes and releases (build, test, npm release, docker push)
- Remove separate docker.yml (consolidated into ci.yml and cd.yml)
- Pin all GitHub Actions to exact SHAs
- Add CI_GITHUB_TOKEN fallback for branch protection bypass

* fix(ci): use single-arch Docker build for PRs

Multi-arch builds with QEMU emulation exhaust runner memory.
Build only linux/amd64 for PRs; full multi-arch in CD workflow.

* test: update multi-arch test to check CD workflow

Multi-arch builds are in cd.yml (releases), not ci.yml (PRs).
CI uses single-arch to avoid QEMU memory issues on runners.
