feat: Refactor to orchestration layer with multi-agent routing and GitHub Marketplace actions (#37)

* Initial plan

* feat: Add orchestration layer with new module structure

- Created orchestrators/ module for multi-agent routing (Ollama/Jules/Cursor)
- Created pipelines/ module for CI resolution and PR lifecycle
- Created actions/ module for GitHub Marketplace actions integration
- Added agentic-triage@^0.2.0 as dependency
- Updated package exports to include new modules
- Updated package description to reflect orchestration focus
- Updated CI workflow to use NPM_TOKEN for publishing
- Updated README with new package name and GitHub Marketplace Actions section
