Remove unused internal methods and improve test coverage

- Remove unused PrefabManager.Get() and Clear() methods
- Remove unused EntityPrefab.Clone() method
- Add test for duplicate WithTag<T>() calls to cover all branches

Per project guidelines, speculative code that isn't used should be
removed rather than maintained. These internal methods were added
"in case they might be useful" but weren't actually consumed.
