✅ test: add pragmatic error path testing for cert package

- Add cert_error_test.go with error path coverage for CreateCertificate
- Test ecdsa.GenerateKey and rand.Int errors via rand.Reader mocking
- Document untestable paths (ECDH conversion, x509.CreateCertificate)
- Improve cert package coverage from 78.9% to 89.5%
- Overall test coverage improved from ~70% to 94.3%

Update analysis documentation:
- Correct outdated test coverage metrics across all docs
- Update implementation score from 8.0 to 8.5/10
- Add comprehensive change history entries
- Fix score inconsistencies between documents
