* [-] fix Pgpool-II support, closes #730
- split `pgpool_stats` into two separate metrics
- update documentation
- update Grafana dashboards
- add `metrics.RowToMeasurement()`
- use `pgx.QueryExecModeSimpleProtocol` for non-Postgres connections
- remove `FetchMetricsPgpool()` and use standard fetching
