fix issues loading the ir from serialized form

there were a number of issues with unhandled errors leaking through
and causing pipelines to fail, and while trying to resolve those by
resuming from the full ir representation encountered numerous issues
where calls to the network when loading idientifiers resurface

also proposed filter_failures to first class pipeline status

the pipeline classes are really bad abstractions that lack a whole
bunch of important composability properties

mitigate a number of issues where foreign json blobs should not be
subjected to the same conversion criteria due to the fact that they
place different semantics on the 'type' key, however had to handle all
known cases because there are some old blobs that we have that do not
have type field set so we cannot short circuit them

extended transitive_paths to be able to exclude arbitrary patterns in
advance of needing to be able to exclude files from indexing and
inclusion for soda etc.
