PolynomialControls (#145)

* start of interpolated phase-wide controls

* interpolated controls now entirely provided by a group, including the indep var comp

* interpolated control group is correctly computing values and rates for scalar controls

* interpolated control group passing all unit tests

* interpolated controls -> polynomial controls.  Added add_polynomial_control method to phase base, and associated PolynomialControlOptionsDictionary.

* Interpolated controls working with gauss lobatto and radau phases.  Need to add support for RungeKuttaPhase and SimulationPhase, and test various things like constraining them and timeseries outputs of their values

* brachistochrone polynomial control rungekutta test passes.  added handling for path constraints of polynomial control values.

* Polynomial controls now respect path and boundary constraints imposed upon them.

* polynomial controls seem to be working everywhere except SimulationPhase.

* moves standard dynamic controls into a modular controls group with their indepvarcomp, eases some of the ordering issues with OptimizerBasedPhase.  Promotes control values and rates at all nodes to the phase level (control_values:u, control_rates:u_rate, control_rates:u_rate2).

* corrections for path constraint options in radau pseudospectral
