Test on most supported major MySQL versions

There is no official Docker image for MySQL 5.0, and I couldn't get any of the
third party ones to work. But the official MySQL and Maria images all work
fine. Like the Postgres version testing, the MySQL versions are tested in its
their own stage, and only on master. The `LIVE_MYSQL_REQUIRED` variable
ensures they run.

Other changes:

* Turn off echoing in databaes engine config scripts.
* Add MYSQL_URL environment variable, so we can easily configure connection
  for testing.
