* Add a prototype hook to supply the site and env to use from TERMINUS_SITE and TERMINUS_ENV environment variables.

* Fix style errors with cbf.

* Remove old copy of SiteEnvLookup

* AI-80: Read TERMINUS_SITE and TERMINUS_ENV from a .env file or environment variables

* Code style

* Add some behat tests that use TERMINUS_SITE and TERMINUS_ENV to select the site

* Remove unused logger from site env lookup hook

* Add some unit tests for SiteEnvLookup

* Code style

* Also test fetching site from repository, and test sites that take just a site, not site.env

* Fix trailing comma in parameter lists. PHP 7.3 accepts this! All other versions give a syntax error.

* Define git user when running on CI

* Only set identity in temporary repository

* Simplify quoting for the benefit of Windows testing.

* Don't clean up in Appveyor (it doesn't work). Fix a couple of minor issues Scrutinizer found.
