Add downstream fragmentation

- Server-side fragmentor mirrors client tactics.
- Refactor fragmentor into common package.
- Add fragmentor unit test, and exercise fragmentor
  in server_test.
- Fix: actually implement one-time coin flip
  as documented in NewTCPFragmentorDialer, which
  was intended to use consistent fragmentation
  per meek session.
