H2C Upgrade client  (#111)

* client: add #upgrade, which allows the h2c client flow: sends the preface, and returns the first stream in half-closed state
* added helper method to generate the HTTP2-Settings header for completing the h2c HTTP/1 flow+
* added tests to the client upgrade method
* added upgrade_client script
