docs: fix stale and inaccurate API docs from the pre-1.0 review

- async: drop the nonexistent `TaskGroup[X]` type parameter from the
  `TaskGroup` doc; document that `return_immediately` and
  `with_timeout_opt` still wait for cancelled children (defer,
  protect_from_cancel) and group defers, and when they return normally.
- double/float: README no longer says the total comparator is deferred;
  the first line of `total_compare` states its NaN-first,
  signed-zeros-equal order.
- debug: README no longer tells users to switch from `@test.assert_eq`
  to `@debug.assert_eq` (they are the same function).
- test: `assert_raise`/`expect_error` failure message now matches the
  async twins ("expected error, but no error is raised");
  `assert_raise_async` points at `expect_error_async`.
- env: document that `set_env_var`/`unset_env_var` do nothing on JS
  without `process.env`.
- `ToJson::to_json` promotion deprecations point at `Json(value)`
  instead of `@json.to_json`; json README diagram likewise.
