Check exit status on passwd/gem install, fix duplicate Usage lines

mo_ubuntu_root_setup's passwd prompt previously replaced a step that
was always mandatory and always run interactively -- folding it into
the curl-piped root phase makes it silently no-op on failure (no exit
check), with the actual breakage surfacing much later and several
layers removed, as a confusing sudo hang inside mo_init_or_migrate_db.
Now fails fast with the recovery steps inline.

mo_gem_install_locked likewise didn't check gem install's exit status,
silently undermining the exact protection its own docstring describes
(pinning bundler/trilogy so bundle install doesn't resolve/compile a
different version) if the install failed.

dev_setup_ubuntu's two header Usage examples were byte-identical --
the second now shows the from-an-existing-checkout invocation,
mirroring dev_setup_macos's header.
