Add a code-comments rule: explain why (only when unclear), one source of truth

Every file under .claude/rules/ is auto-loaded into context each Claude
session, so this makes the convention apply to all sessions for all
developers: comment the non-obvious *why*, never restate the *what*, and
when a rationale is worth writing put it once at the load-bearing site
with pointers elsewhere -- don't duplicate the same explanation across
files, where it only multiplies the staleness surface.
