feat(ui): implement Phase 5 - Slash Commands

- Create yoker/ui/commands/ directory with command registry
- Add Command, CommandHandler, and CommandRegistry types
- Move /help, /think, /skills, /context to UI layer
- Create /<skill-name> skill invocation command
- Add Agent.inject_skill_context() method
- Update __main__.py to use command registry
- Add comprehensive command tests
