fix(daemon): stable Codex bridge key across OAuth token refresh (#2099)

The bridge key previously included the raw OAuth access token, so every
refresh rotated the key and the cleanup loop killed the running bridge
while the SDK subprocess still pointed at its port. Use a stable key
(account + FedRAMP flag for OAuth; hashed API key) so the bridge server
survives token rotations and only restarts when the auth identity itself
changes.
