build: replace markdownlint-cli2 with mado (#57)

* build: replace markdownlint-cli2 with mado

Swap the Markdown linter from markdownlint-cli2 to mado in the pre-commit hook, setup script, and docs. Port the config to mado.toml: drop MD013 (line length) and MD024 (duplicate headings), exclude CHANGELOG.md, and set MD029 to "ordered" so the README install steps stay numbered. Add the blank lines mado requires around the mix.exs code block.

* ci: add mado markdown lint job

Run `mado check` on pull requests and pushes to main, and add it to the required-jobs gate. Markdown linting previously ran only in the local pre-commit hook.

* chore(build): remove CHANGELOG specific rule and comment
