feat: add Forge completed-task automation

Count-based Forge automation so recurring Forge scopes automatically draft a new episode after a configurable number of completed relevant tasks (default 10). Includes overlap prevention, requeue cap, cursor fallback, non-recurring guard, evidence feedback loop prevention, and UI debounce.
