feat: migrate MarkdownRenderer to remark/rehype with KaTeX and Mermaid (#2022)

Migrate chat markdown rendering from `marked` to the unified/remark/rehype pipeline. Adds KaTeX LaTeX rendering (49586..49586), Mermaid diagram support, lazy-loaded modules with promise caching and RAF debouncing, and a comprehensive HTML preprocessor (escapeRawHtmlBlocks) for safe inline/block HTML passthrough with code span tracking, multiline HTML block collection, and URI scheme validation.
