* feat: add QA.md discovery to QA node prompt

Instruct QA agents to look for project-level QA instructions before running checks, and follow any found alongside standard validation.

* docs: add project QA instructions

Add QA.md with dev server startup, lifecycle discipline, Playwright UI testing,
backend/API testing, and artifact recording guidance for QA agents.

* docs: use Playwright skill for interactive QA browser testing

Replace make run-e2e with /playwright skill since e2e suite is for CI,
not QA agent interactive testing.

* fix: trust base QA policy in QA prompt

Ensure QA policy discovery uses base-branch instructions, not mutable PR worktree content, and remove the QA.md-only fast path.

* fix: align QA instructions with prompt

Use task-ID DB naming and include pr_url in QA result artifact guidance so QA.md matches the workflow prompt contract.
