:bug: Fix thinking blocks multiplying during stream + add planck_cli README

- chat_component: thinking_delta was using a unique_integer id for the entry
  but looking it up by "think-#{aid}" — mismatch caused every delta to create
  a new block instead of updating the existing one; fix uses "think-#{aid}"
  as the stable id for both creation and lookup
- planck_cli/README.md: document how to run with --sname for sidecar support
