:sparkles: AGENTS.md prepended to all agents — orchestrator, static and dynamic workers

Previously only the orchestrator received AGENTS.md. Workers are the
implementers most likely to need project conventions, so now all agents
start with the same context:

- Tools.prepend_agents_md/2 made public in planck_agent — single
  implementation shared by headless startup and spawn_agent at runtime
- orchestrator_tools/7 and spawn_agent/6 accept a cwd parameter so the
  working directory is captured in the tool closure at team-materialisation
  time; dynamic workers spawned later get AGENTS.md prepended at that point
- start_workers in planck_headless calls prepend_agents_md for each static
  worker, mirroring what start_orchestrator already did
- Agent.t gains a cwd field (populated from start opts) for observability
- Private prepend_agents_md/find_agents_md removed from planck_headless
- Tests: prepend_agents_md/2 unit tests + session_lifecycle integration
  tests for static worker AGENTS.md prepending and cwd state storage
- specs/planck-headless.md updated to document the two-layer prompt assembly
