Fix stuck MCP session: repair missing agent_session_id from sub-session id (#1699)

Workflow sub-session ids embed the node execution id (`:exec:<id>`).
When a spawn race or daemon restart left `agent_session_id` null on the
NodeExecution row, MCP self-heal and rehydration both failed to resolve
the execution — hard-blocking the session with no recovery path.

Extract the embedded execution id as a fallback lookup in both
`rehydrateSubSession` and `mcpSelfHeal`, persisting the repaired value
so subsequent lookups succeed without re-parsing.

Also make `getByAgentSessionId` prefer active executions when a reused
session appears on multiple rows, so self-heal rebuilds node-agent with
the current node context.
