docs(plan): late-arrival handling, per-definition cleanup, parallel rebuilds with periodic flag

Fixes three concerns from the deep review:

1. Late-arriving enablers — when an incremental cycle's INSERTs add a
new admin grant, validated attachment, or role declaration, the
downstream tier INSERTs re-run once without the load-number filter so
pre-enabler candidates can catch up.

2. npas:<spaceRef> invalidation cleanup — per-contributor triples
(signer, pubkey, created, root-admin seeds) move to a dedicated
npadef:<artifactCode> npa:SpaceDefinition entry with npa:viaNanopub.
Standard DELETE-on-invalidation now cleans root retractions and
supersessions correctly; the aggregate npas:<spaceRef> keeps only
contributor-independent space-identity triples.

3. Sticky attachment staleness — space-state graph IRI becomes
npass:<trustStateHash>_<loadCounterAtBuildStart>, so a full rebuild
always writes to a fresh graph and readers stay on the old one until
an atomic pointer flip. An npa:needsFullRebuild flag is set when the
incremental path DELETEs structural derivations (admin grants,
attachments, role declarations); a periodic worker (~5-15 min) checks
the flag and runs a full rebuild in parallel with reads. Trust-state
flips reuse the same rebuild path.
