docs(plan): drop v2 suffix; the v2 plan is now the plan

Replaces the obsolete v1 plan with the current design. The old per-space
graph + materializer/validator split is gone; the live document is the
add-only npa:spacesGraph + npass:<trustStateHash> incremental design.
