docs: revise plan to workflow-declared post-approval agent routing (#1612)

Pivot away from the narrow `merge_pr` MCP-tool-on-Task-Agent approach
toward a workflow-declared post-approval agent routing model. Workflow
templates now declare `postApproval: { targetAgent, instructions }`;
Task Agent spawns that agent with interpolated instructions after human
approval. Eligible targets: workflow-declared agents + `task-agent`
only.

Adds a new `approved` task status (lifecycle:
`in_progress → approved → done`) and documents the full end-node handoff
protocol, template grammar, validator rules, 5-PR breakdown, and open
questions. Revision history captured in §8.
