feat(web): compact approval banners into one-line + modal pattern (#1575)

Both PendingTaskCompletionBanner and PendingCompletionActionBanner now
render as compact one-line status indicators (icon + brief label +
action buttons) instead of large inline cards.

Full details (rationale, reported summary, action script/instruction/
mcp args, approval note textarea) are deferred into modals opened by
the Approve or Reject/Send-back buttons, keeping the task thread view
uncluttered while preserving all existing functionality and data-testid
attributes.

Updated PendingCompletionActionBanner tests to match the modal-based
flow (approve/reject confirmation now requires an extra modal-confirm
step). Pre-existing test failures (ChatContainer, SlashCommandOutput,
SpaceIsland) are unrelated to this change and reproduce on the base
branch.
