Support per-file visibility filter in templated files config

Files entries can now be an object with `content` and `visibility`
fields instead of a plain string. When `visibility` is set to
`public`, `private`, or `internal`, the file is only included in
the templated-files PR for repos matching that visibility. Plain
string entries continue to work and apply to all repos.

Example:
  files:
    'LICENSE':
      content: |
        MIT License...
      visibility: public
    'SECURITY.md': |
      always applied
