refactor: make coordinator delegate goals, not solutions (#174)

Rewrite coordinator and specialist agent prompts to enforce goal-oriented
delegation. The coordinator was sending entire file contents and step-by-step
instructions to sub-agents, making them copy-paste machines instead of
autonomous specialists.

Changes:
- Add explicit Delegation Rules to coordinator prompt with bad/good examples
- Reframe coder from executor ("no more, no less") to autonomous implementer
- Add goal alignment checking to reviewer prompt
- Reinforce autonomous investigation in debugger and tester prompts
- Remove redundant Executor agent (built-in Bash agent covers this)
- Update all tests for 7 agents instead of 8
