Split src/config.ts into modular config/ directory

Extract the 911-line monolithic config.ts into focused modules:
- config/schema.ts: Zod schemas, types, constants, resolveHooks/resolveMcpServers
- config/merge.ts: config merge helpers, getMergedConfig, readConfig
- config/io.ts: file I/O (read/write/find config files)
- config/expand.ts: env/path/volume expansion utilities
- config/index.ts: barrel re-export
- git.ts: getGitRoot, getGitWorktreeParentPath
- ssh/known-hosts.ts: getSshKeys, getSshHosts, getFilteredKnownHosts
