feat: add GLM-5 flagship model support, remove deprecated GLM-4.7

- Add GLM-5 (745B params, 200K context) as new flagship model
- Remove all GLM-4.7 references from source code and tests
- Update all tier mappings to use glm-5
- Update title generation to use glm-5
- Update 61 test references from glm-4.7 to glm-5

All 22 GLM provider unit tests pass
