Rename InatImportNew to InatImport and fix checkbox state on go-back

Rename FormObject::InatImportNew to FormObject::InatImport and
Components::InatImportNewForm to Components::InatImportForm for
cleaner naming. Updates all field IDs, param keys, and test selectors.

Fix Import All checkbox incorrectly showing as checked after going
back from the confirmation page. Superform's hidden input sends "0"
for unchecked checkboxes, but "0" is truthy in Ruby so the checked
attribute was always rendered. Normalize checkbox values to nil
unless explicitly "1".
