* ci: add Coveralls alongside Codecov for coverage comparison

Set up parallel coverage reporting to both Codecov and Coveralls to evaluate which service better handles monorepo coverage merging.

Changes:
- Add .coveralls.yml configuration with daemon/web flags
- Update CI workflow to upload coverage to both services
- Add coveralls-finalize job to merge parallel uploads

Both services run in parallel without impacting CI performance. This allows side-by-side comparison of coverage merging accuracy.

* ci: add coverage reporting for offline integration tests

Enable coverage collection and reporting for all matrixized offline test modules (agent, session, rpc, database, filesystem, websocket, git, components, mcp).

Changes:
- Add --coverage flag to offline test runs
- Upload coverage from each module to Codecov and Coveralls
- Update finalization job to wait for offline test coverage
- Add flag definitions for all offline modules in codecov.yml and .coveralls.yml

This provides complete coverage visibility across unit, integration, and web tests.

Complete migration to Coveralls for all coverage reporting with comprehensive test coverage across all test suites.

Changes:
- Add coverage collection for online integration tests (11 modules)
- Remove all Codecov uploads and configuration
- Delete codecov.yml
- Update Coveralls finalization to include online tests
- Add online test flags to .coveralls.yml
- Update README.md badge from Codecov to Coveralls

Coverage now includes:
- Unit tests (daemon + shared, web)
- Offline integration tests (9 modules)
- Online integration tests (11 modules)
Total: 22 separate coverage sources merged by Coveralls
