fix(mappings): ensure schema completeness across all modules

This commit ensures all Elasticsearch mappings are complete and
consistent with their JSON schemas across multiple modules.

1. operation_logs mapping updates:
   - Add missing field: ill_request.library_pid (keyword)
   - Add missing field: loan.note (text)
   - Add complete scan object with transaction_location,
     pickup_location, item (with full nested structure), and note
   - Move file object from loan to top-level properties (schema
     alignment)

2. patrons mapping updates:
   - Add missing field: source (keyword)
   - Source tracks if record was loaded in a batch

3. notifications mapping updates:
   - Add missing field: reminder_counter (integer)
   - Current reminder count at top level

4. remote_entities mapping updates:
   - Add missing field: id (keyword)
   - Library ID field

All modules now have 100% schema-to-mapping coverage, ensuring
proper indexing and searchability of all defined fields.
