fix: align migrations with production state (mostly no-op) DEV-1272 (#6598)

### 💭 Notes

This PR updates existing migrations to ensure they are fully aligned
with the current production migration state.
No database changes are performed by these migrations, except for the
trackers app.

The only migration that performs actual database operations is:

- kobo/apps/trackers/migrations/0007_alter_nlpusagecounter_indexes

This migration:

- Creates missing indexes
- Cleans up legacy constraints safely
- Updates the migration state to match the real production schema
