Remove premature Bootstrap migration infrastructure

This commit removes Bootstrap migration documentation and infrastructure
that was created prematurely in Phase 6. Per the comprehensive modernization
plan, Bootstrap migration should happen AFTER Phlex components are built.

**What Was Removed:**
- doc/BOOTSTRAP_MIGRATION_GUIDE.md - Comprehensive migration guide
- doc/BOOTSTRAP_MIGRATION_README.md - Migration documentation index
- doc/BOOTSTRAP4_MIGRATION_CHECKLIST.md - 400+ item checklist
- app/assets/stylesheets/mushroom_observer_migration.scss - Toggle stylesheet

**Why Removed:**
These files were based on incorrect assumptions about "parallel systems"
(Bootstrap 3 and 4 gems cannot coexist) and created before they are needed.
Per the comprehensive plan (doc/Bootstrap Upgrade Plan for MO.md):

- Phase 2 (weeks 4-7): Build Phlex components FIRST
- Phase 3 (weeks 8-11): THEN create BS4 compatibility
- Phase 4 (weeks 12-14): THEN migrate to Bootstrap 4

Current status: Phase 2 (Phlex) has not started yet.

**What Was Kept:**
- app/assets/stylesheets/mo/_map_theme_to_bootstrap4.scss (for future use)
- doc/BOOTSTRAP4_THEME_MAPPING.md (reference documentation)
- doc/BOOTSTRAP4_BREAKING_CHANGES.md (reference documentation)

**What Was Added:**
- doc/BOOTSTRAP_FUTURE_WORK.md - Explains when BS work will happen
- doc/THEME_STANDARDIZATION_SUMMARY.md - Completion summary for Phases 1-5

**Updated:**
- app/assets/stylesheets/mo/_map_theme_to_bootstrap4.scss header (future use)
- PR #3629 description (corrected scope)

**Current State:**
Theme standardization (Phases 1-5) is COMPLETE ✅
Next focus: Phlex component development (Phase 2)
