Add additional edge case tests for entity hierarchy coverage

Add 13 new tests to improve code coverage for hierarchy functionality:
- SetParent to Entity.Null when no existing parent
- Last child removal cleanup in RemoveChild and Despawn
- Entity with no hierarchy despawn handling
- GetChildren/GetAncestors/GetRoot with despawned entities
- GetDescendants with despawned grandchildren
- Parent change cleanup for empty children sets
- DespawnRecursive with balanced tree hierarchy
- CleanupEntityHierarchy with multiple children

These tests ensure all code paths in the hierarchy implementation
are exercised, particularly cleanup logic for empty collections.
