Raise exception not panic on bad `ConsolidateBlocks` input (#15110)

This doesn't change the fact that the input is incorrect, but we
shouldn't be making Rust-space panicking assertions on invalid inputs
that are fed in (potentially) from the user.  We can do that if it would
_purely_ be an internal logic error from within Qiskit, but this case
can just be a legitimate user mistake.
