Fix CUDA shader object writing (#548)

* Fix subtle bug in shader object data writing

We always need to write sub-objects to global device memory, even if the parent shader objecct represents an entry-point.

* Zero initialize acceleration structure handle

This doesn't really affect anything, but a zero handle makes more sense to see during debugging when an acceleration isn't built yet.
