feat(instanceHistory): add root_uuid to instancehistory DEV-754 (#6143)

### 💭 Notes
Add root_uuid to InstanceHistory when editing an instance. No
user-facing behavior change.

Make sure to run migrations against kobocat

1. ℹ️ have an account and a project with at least one submission
2. Get the instance id of a submission
3. Edit the submission
4. In a Django shell, get the most recent InstanceHistory object
5. 🟢 Make sure the root_uuid has been set and matches the root_uuid of
the Instance
