fix(projectHistoryLogs): fix 500 on updating QA on modified instances DEV-595 (#5845)

### 📣 Summary
Fix a bug that was causing errors when trying to update NLP data on
submissions that had previously been edited.

### 💭 Notes
Look up instances by `root_uuid` instead of by `uuid`. Based off
