Further revisions to unifier quality thresholds (#86)

Trifurcate `unifier_config::min_quality` to `min_AQ1`, `min_AQ2`, and `min_GQ`.

The AQ thresholds control adjudication of allele existence. The unifier includes alleles having any
observation with AQ > min_AQ1, or having more than one observation each with AQ > min_AQ2 (min_AQ1 >= min_AQ2).

The GQ threshold affects estimates of allele copy number. Genotype calls with GQ >= min_GQ count towards the copy number estimates of the constituent alleles.
