fix unique constraint on nullable column for table variable & nulls ordering for primary key indexes (#2562) (#2586)

UNIQUE constraints declared on pltsql table variables should only allow one null value by using the PostgreSQL nulls not distinct behavior. For other relations we already fixed this behavior here #2392

Also fixed the default nulls ordering for pltsql primary key constraints to ASC NULLS FIRSTS & DESC NULLS LAST.
