add request-logger, winston transports to config

adds the winston transports / config to the configuration files so that
they made be changed / overridden in environments.

updates the tests to assert better, reflect these changes, updated
winston module

request-logger adds request-id, method, url, duration line for requests
to eventually replace koa-logger. this will allow a better format to
reach logstash / bunyan-like logging flows.
