refactor(git): introduce Service as single public API

- rename workflow.go → service.go with Service type as public entry point
- all git operations now methods on Service (CreateBranchForPlan, MovePlanToCompleted, etc.)
- keep repo type internal/unexported
- add handling for already-moved plan files in MovePlanToCompleted
- update CLAUDE.md with git package API documentation
