feat(processor): graceful prompt variable handling and review improvements (#16)

* feat(processor): add fallback text for empty PLAN_FILE and PROGRESS_FILE

Add getPlanFileRef() and getProgressFileRef() helpers that return
descriptive fallback text when running in review-only mode without
a plan file, preventing broken/confusing prompts.

- getPlanFileRef: returns "(no plan file - reviewing current branch)"
- getProgressFileRef: returns "(no progress file available)"
- Remove progressPath parameter from replacePromptVariables

* docs(prompts): add commit history to review prompts

Include `git log master..HEAD --oneline` alongside the diff command
so reviewers can see implementation context before reviewing code.

* fix: wire ProgressPath to processor config in createRunner

The ProgressPath field was added to processor.Config but never set in
createRunner(), causing {{PROGRESS_FILE}} to always expand to fallback
text instead of the actual progress file path.

* fix: address codex review findings

rename review section labels for consistency with numbered review phases
