Updates based on code review

- Change root path to "/streaming"
- Remove ChannelsWithSubsz and instead add []*Channelz with omitempty
  in Channelsz
- Updated tests accordingly
- Remove wait for http handler (since it should avail at this point)
- Use http.Error() where appropriate
- Extract getting offset/limit and setting min/max into 2 functions,
  which, along with ChannelsWithSubsz suppression simplied the
  handleChannelsz function.
- Added README section on Monitoring (may need updating when we
  settle on what to do with the root path)
